Title: Licensed Mental Health Therapist
JOB SUMMARY
Individual will be responsible for providing diagnostic assessment, treatment planning and therapy services to adolescents, young adults and their families at the Hopkins clinic. Responsibilities include therapy with clients and family, parent education, possible group therapy, engaging in outreach and community education activities and related paperwork. This position will include time spent on outreach and networking activities to build community knowledge and client base. This position requires the following hours:  
  • Part time, 20 hours a week position – this position is benefits eligible
    • Two Saturdays a month 9:45 – 2:00
    • Mondays 11:45 – 8:00
    • Tuesdays 11:45 – 5:00 
    • Thursdays 11:45 – 5:00 
ESSENTIAL JOB FUNCTIONS 
  • Provide direct therapy services to adolescents, young adults and their families at the Hopkins clinic 
  • Provide outreach, consultation and other professional presentations to school systems, referral sources and other related community groups
  • Must have reliable transportation and a valid MN State Driver’s License to travel and provide off site mental health services at Satellite clinics or local schools 
  • Build referral network through community meetings and outreach activities 
  • Carry out all necessary administrative activities regarding charting, billing, program reporting and scheduling 
  • Attend scheduled Mental Health program, all-clinic and all-staff meetings on the second Thursday of each month 
QUALIFICATIONS 
Required:
  • Master’s degree in Social Work, Marriage and Family Therapy or Psychology from an accredited University 
  • Previous experience working as a Licensed Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Psychologist (LP) in the state of MN
  • Previous experience working with adolescents and young adults
  • Demonstrated knowledge of age related growth and development principles
  • Demonstrated ability to develop and maintain therapeutic relationships with adolescents and young adults
  • Ability to build and sustain effective collaborative relationships with colleagues, staff and community partners
  • Ability to work evenings and some weekends 
  • Ability to work effectively with individuals from a variety of economic, social and culturally diverse backgrounds
  • Technical proficiency in Excel, Word and Outlook
  • Excellent verbal and written communication skills
  • Committed to the mission, objectives and core values of myHealth 
Preferred: 
  • Experience using Electronic Medical Records (EMR)
  • Experience in non-profit healthcare, reproductive health or adolescent health issues
  • Credentialed provider – able to bill insurance for services REQUIRED LICENSES/CERTIFICATIONS Currently licensed as an independent clinical social worker (LICSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC) or Licensed Psychologist (LP) in the state of MN 
PHYSICAL ENVIRONMENT/WORKING CONDITIONS Environmental/Working Conditions: Work is performed primarily in a clinic environment. 

Physical Demands: May require occasional lifting of supplies and equipment of up to 50 lbs.

Title: Becoming Program Manager
Reports to: Director of Clinical Services

JOB SUMMARY
Supervise Becoming Program and provide direct comprehensive community health services to pregnant and parenting 12-23 year old young women and their families who are eligible for the program.
  
Hours: 20 hours/week – this is a benefits eligible position 

REPORTING RELATIONSHIPS 
Directly supervise one home visiting nurse

ESSENTIAL JOB FUNCTIONS 
  • Provide prenatal and postnatal home visits incorporating assessment, monitoring and education of physiological status, nutritional and health behaviors, psychosocial needs, home environment, feeding, child spacing and pregnancy prevention options, parenting skills, infant care and developmental milestones
  • Supervise Becoming Program activities, and program support staff. This includes providing feedback and annual reviews for support staff. Review and sign time cards every two weeks and mileage form monthly
  • Must have reliable transportation, a valid Minnesota state driver’s license and valid auto insurance to travel and provide home health visits in Carver, Western Hennepin and Scott counties
  • Attend local and state coalitions as a representative for myHealth
  • Document patient care and enter into database. Provide information for grant writing/reporting as requested
  • Participate and lead setup and coordination of Young Mom’s Connect group and cooking class
QUALIFICATIONS
Required:
  • Graduate of an accredited school of nursing
  • Minimum of 2 years of nursing experience in adolescent health care 
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to access consultants as needed
  • Ability to work effectively with individuals from a variety of economic, social and culturally diverse backgrounds
  • Technical proficiency in Excel, Word and Outlook
  • Excellent verbal and written communication skills
  • Committed to the mission of myHealth
Preferred:
  • Training and experience in working in a community based health care setting
  • Experience with EMR systems 
  • Leadership and supervisory skills, utilizing good judgment and problem solving skills
  • Experience in non-profit health care, reproductive health or adolescent health issues

REQUIRED LICENSES/CERTIFICATIONS 
  • Possess current and valid Minnesota Nursing License
  • Current CPR certification
  • Must have own transportation, valid MN driver’s license and valid auto insurance to travel between clinics and to client home visits
  • Current Public Health Nurse Certification or must be willing to become certified within the first month of hire

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT 
Environmental/Working Conditions: Work is performed in a community and office environment. Work may be stressful at times due to a busy caseload. Interaction with others is constant. 

Physical Demands: Work may require hand dexterity for telephone and office machine operation. Position may require occasional lifting of supplies and equipment up to 20 pounds in and out of car.

Applications may be sent year-round. A minimum commitment of three months is required for most long-term positions. Some positions may require a longer time commitment.
